Arrange a Care Conversation

An appointment request is the start of a plan, not a promise that a clinician will arrive unannounced. After you write, a coordinator reads the note and replies by email with a time for a first conversation.

Please tell us who the care is for, what has changed, and which part of the day is hardest. Useful details include falls, appetite, memory, breathlessness, medicines that are hard to track, and whether a spouse is carrying more than they can.

What happens next

  • We reply by email to the address you provide.
  • The first conversation can include the older adult and one family member.
  • If a home visit is right, we agree the window before anyone travels.
  • Urgent danger is not handled through this form. Contact local emergency services.
